By forming alliances with Medicare-certified agencies, we can deliver

Skilled-level Nursing services with Home Plus dignity – and typically at less cost to patients and families.

CLINICAL ADVANTAGE: MAXIMUM 1 to 6 STAFFING RATIO

Unlike large facilities with 1:20 staff ratios, our Home Plus typically maintains a 1:6 staff to patient ratio.  With more direct patient contact and interaction, we can detect subtle changes in condition (UTI, sepsis, pneumonia) days or hours earlier compared to larger, lesser staffed facilities – and prevent re-admissions.

• Licensed Nurse on staff 24 hours/day, 7 days/week
• All Private Rooms, all Private Bathrooms. Extra large roll-in ADA compliant showers
• No restrictive institutional schedules
• Licensed Dietician: Will accommodate any dietary preference or restriction
• No Mandatory dosage reductions. Home Plus is not required by CMS to reduce or eliminate medications that are safe and effective and working for the resident
• Rapid Response: Same-day assessments if needed and same-day admissions in emergency
• Focused on Zero Readmissions: High-frequency rounding/monitoring ensures medication adherence/compliance, hydration and fall prevention
• Infection Control: Lower risk of infections. All residents occupy Private Suites, significantly reducing cross-contamination risks compared to SNFs

COMPLEX NURSING

Through our licensed Home Health Affiliates, we accept and manage:

• Wound Care: Stage 3-4 pressure ulcers, surgical wounds and Wound Vac management
• IV Therapy: Administration of antibiotics and hydration (agency dependent)
• Respiratory: O2 management and COPD monitoring
• Diabetes: Insulin management including sliding scales
• Enteral Feeding: Liquid nutrition management
• Catheters: Indwelling (Foley) and Suprapubic and urostomy care
• Colostomy: Full ostomy management. Plus pre-op and post-op care and support
• Cardiac & Pulmonary Rehab: Comprehensive recovery programs - heart attack, heart surgery, or chronic lung diseases, COPD
• Medical Nutrition Therapy: Specifically for diabetes or renal disease

HEAVY MOBILITY & ADL SUPPORT

• Transfer Status: Sit-to-stand, full body and mechanical lift transfers
• Feeding: 1:1 feeding assistance for dysphagia or cognitive decline
• Incontinence: Full bowel/bladder incontinence management

ON-SITE REHAB | MEDICARE PART B

Our Micro-SNF provides PT, OT, SP through Medicare-certified providers to ensure smooth transitions and reduced readmissions.

• Physical Therapy: Gait training, post-stroke mobility and strengthening
• Occupational Therapy: ADL retraining and adaptive equipment fitting
• Speech Therapy: Swallowing (dysphagia) assessments and cognitive therapy

NO SECONDARY DISCHARGE PLANNING REQUIRED

If short-term rehab placement evolves into long-term needs, the resident is “already home”  avoiding secondary discharge hassles.

We partner closely with hospital case managers and discharge planners to reduce readmissions, support recovery, and ensure a smooth, timely transition

from hospital to the next level of care.
